Purpose
This agreement must be completed for participation in any youth ministry activities, programs, or trips sponsored by Fayetteville Community Church (hereafter referred to as “the Church”), whether these activities take place on or off church property.
Acknowledgment and Assumption of Risk
I understand that participation in church-sponsored youth activities may involve risks, including but not limited to, transportation, recreation, physical activities, or unforeseen events. I acknowledge that the Church, its staff, and volunteers will take reasonable precautions to promote the safety and well-being of all participants, but that injuries or accidents may still occur. I willingly assume all risks associated with my child’s participation in these activities.
Release and Hold Harmless Agreement
In consideration for my child being allowed to participate in youth activities sponsored by Fayetteville Community Church, I hereby release and discharge Fayetteville Community Church, its pastors, officers, staff, employees, volunteers, and representatives from any and all liability, claims, demands, or causes of action arising from or related to any loss, damage, or injury (including death) that may occur to my child while participating in these activities, whether on or off church property. I also agree to indemnify and hold harmless Fayetteville Community Church, its staff, and volunteers from any liability, costs, or expenses incurred as a result of my child’s participation or behavior during these events.
Medical Authorization
In the event of illness or injury, I authorize any adult leader representing Fayetteville Community Church to administer basic first aid and to seek medical care for my child as deemed necessary by a licensed healthcare professional. I understand that every effort will be made to contact me prior to such treatment. I accept responsibility for all medical expenses incurred.
